Uploading Vaccination Records

Immunization Deadline is Sept 12 ,2025

Complete your immunizations by the first day of classes. If your immunization records are incomplete by September 12, 2025, a registration hold and a $50 fine will be applied to your student account.

If you are unable to do so, you must attend the immunization fair held during the first week of classes.

Immunization Documentation Examples

Please note that records must show your name and date of birth in order to be accepted. Screenshots for health portal records are accepted. For records in languages other than English, translations into English must be provided.

Please note that lab reports should include the following information to verify:

a.Patient’s Full Name
b.Patient’s Date of Birth
c.Name of Test Performed
d.Date of Test
e.Test Result Numeric Values (cannot just say “positive” or “negative”)
f. Test Reference Ranges
g.Ordering Provider Name
